Family/Group Registration Form – Art of Mentoring 2017
Step 1: Submit a separate registration form for each member of your group.
- Each registration should take you about 10 minutes.
- If you are requesting scholarship funds, a family discount, or you are a part of a regional group discount, make sure to note that in the registration form.
- Your spots are not reserved until we receive a registration and a deposit for every member of your group.

Step 2: Pay. Make a single payment for all the people you are registering.
- Your spots are reserved once we have received a deposit of $100 for each registration you have submitted.
- You can pay in full now, or pay just the deposit. Final balance and medical forms for your whole group are due Aug 1.
- If any member of your group requested a scholarship: You must pay the $100 deposit, but it is refundable, if the person chooses not to accept the scholarship funds offered.
