Family/Group Registration Form – Art of Mentoring 2017

Step 1:  Submit a separate registration form for each member of your group.

Step 2: Pay.  Make a single payment for all the people you are registering.

  • Your spots are reserved once we have received a deposit of $100 for each registration you have submitted.
  • You can pay in full now, or pay just the deposit.  Final balance and medical forms for your whole group are due Aug 1.
  • If any member of your group requested a scholarship: You must pay the $100 deposit, but it is refundable, if the person chooses not to accept the scholarship funds offered.

      • Payable to: “Vermont Wilderness School”  /   Memo line: “AoM”
    • To pay by Credit Card or PayPal
